Barcodes Added to VRS Forms to Prepare for Modernization

VRS has added barcodes to the bottom of the first page of about 40 forms, including Minnesota Life forms. Including the barcode on the forms, part of the agency’s preparation for Modernization, will enable VRS to electronically sort the forms when they arrive, therefore reducing processing time and improving customer service.
Please also note that certification statements on the Request for Refund (VRS-3) 
47kb have moved to a separate page. When you print the VRS-3, please make sure you have a copy of the entire form for your employees and that they understand the consequences of taking a refund. Also be sure to provide a copy of the IRS 402(f) Special Tax Notice 28.5kb, which outlines tax information your employees need to know before taking a refund.
Reminder: VRS updates forms frequently, so please print them on an as-needed basis to ensure you have the most current version. Outdated forms may take longer to process and in some cases may have to be resubmitted. As VRS continues to implement Plan 2 legislation and Modernization, forms will continue to change.
