You may choose one of four options at one, two, three or four times your compensation, not to exceed $700,000.
Depending on the option you choose for yourself, you can cover your spouse for up to half the maximum amount of your coverage, not to exceed $350,000. Coverage ends for your spouse when your coverage ends or if you and your spouse divorce. If both you and your spouse are eligible to participate in the Optional Group Life Insurance Program, neither of you can buy additional coverage for the other.
You can cover each dependent child who is at least 15 days old for $10,000, $20,000 or $30,000, depending on the coverage option you select for yourself. Coverage ends when your coverage ends or when your child turns age 21 or, if before age 21, becomes self-supporting or marries. If your child has a physical or mental disability and depends primarily on you for financial support, coverage may continue for up to three months after he or she is no longer disabled.
Proof of good health (evidence of insurability) is not required if you enroll in the Optional Group Life Insurance Program within 31 days of becoming employed in an eligible position or a qualifying event, such as marriage or the birth or adoption of a child. Proof of good health is required if:
- You apply after 31 days of your employment date or a qualifying event.
- You wish to add your spouse or dependent child to your coverage after 31 days of your employment date.
- You wish to purchase more than $350,000 for yourself.
- You wish to increase your optional group life insurance coverage for yourself or your spouse.
