How to Read Your 1099-R
- Box 1. Gross Distribution Benefits VRS paid to you in 2011. Includes the total of your monthly benefit, any cost-of-living increases and any supplements you were paid.
- Box 2a. Taxable Amount Total taxable amount of your benefit. Equals the amount in Box 1 minus the amount in Box 5.
- Box 4. Federal Income Tax Withheld Total amount of federal income tax withheld in 2011. Based on the total number of exemptions reported on your Request for Income Tax Withholding (VRS-15).
- Box 5. Employee contributions/Designated Roth contributions or insurance premiums Amount of your benefit that is not subject to tax. Includes any contributions you made to VRS on which you already paid income taxes. Subtract the amount in Box 2a (taxable amount) from the amount in Box 1 (gross distribution) for the amount shown in this box.
- Box 12. State Tax Withheld Total amount of state taxes withheld if you lived in Virginia in 2011. Based on the total number of exemptions reported on your Request for Income Tax Withholding (VRS-15).
- Health Insurance Premiums If you are eligible to receive a health credit, the amount shown in Health Insurance Premiums is the difference between the total health insurance premium amount and health credit amount. If you do not receive a health credit, the amount in Health Insurance Premiums reflects the total amount of your health insurance premiums.
